- Soil Erosion & Sedimentation Control Permit Application - The Provisions of Part 91 of Soil Erosion & Sedimentation Control of 1994, PA 451, as amended, requires you to have a permit to fill, dig or grade within 500 feet of a lake, stream or waterway in the State of Michigan. The Permit is the responsibility of the property-owner or designated agent (contractor) and must be issued by our office before any work has started.

- Current Spouse Release Form - This form should be used when a joint tax filing has been made and a current spouse wishes to waive any right to a portion of the tax refund so that the entire tax refund can be applied to a past due child support obligation owed by the other spouse.
